The third parties where we use to provide our services such as website hosting, emails exchange, prescription supplier and payment system … etc, the providers have assured their complicity with the GDPR.
Your personal data where we collect, such as telephone enquiry, patient forms and during the appointment of consultation and treatment … etc, we do only use it for providing our services to you and strictly comply with GDPR:
We use your contact information to arrange bookings for our services, prescription orders, payments, reminders and clinic updates … etc.
We use your medical history and lifestyle information to form a diagnosis, treatment plan and health advice, and also to formulate prescriptions.
Retention and disclosure of your personal data
For legal purposes, we are obliged to keep written records of every appointment of consultation and treatment. Such records will be retained for 8 years after your most recent appointment. You can request to have your record deleted after this period. Otherwise, we may retain your record for a longer period to provide a better service for your future possible return.
Your records are strictly confidential and will not be disclosed to a third party in normal circumstances without your written consent. However, if there are any exceptional circumstances for example we need to comply with the law. We will inform you wherever possible.

You can obtain a copy of your record with us by writing to davy@longevityclinicuk.co.uk. A fee will be involved for copies of your record. However, repetitive or excessive requests maybe declined.
